INTRODUCTION
To properly treat asthma it is important to classify a patient's current severity to determine the appropriate therapy choices; therefore a stepwise approach is used. Asthma is classified as either mild intermittent or persistent. Persistent asthma is further classified as mild, moderate or severe. Regardless of the classification there may be periodic exacerbations ranging from mild to severe which can make therapy quite challenging and requires the patient, patient's family and physician to watch closely for any changes. Even a mild intermittent asthmatic can have severe life threatening episodes. These episodes may be separated by months or years with no symptoms at all. A patient's asthma classification certainly can and probably will change (in either direction) over time so just one symptom characteristic of a given classification level is enough to raise a patient into that severity class thereby providing the best control possible. Due to the overlapping nature of the categories if the classification seems a bit fuzzy, the patient should be staged in the highest class for which any characteristics are seen.

LUNG FUNCTION TESTING
Before beginning a discussion of the classification of asthma it is important to understand the common tests that are used to determine a patient's level of pulmonary function. It is very helpful to perform at least a basic pulmonary function test and not base a diagnosis of the severity (type) of asthma only on the signs and symptoms presented. The most common office test is spirometry which measures the maximal volume of air forced on exhalation from the point of maximum inhalation (forced vital capacity (FVC) and the volume of air exhaled during the first second of the FVC (FEV1). A patient can also use a peak flow meter at home to check the peak expiratory flow (PEF) variation between morning and in the afternoon (after using a short acting beta-agonist inhaler) to get the PEV variability. To determine the predicted PEF get a peak flow reading in the afternoon when the patient feels as close to normal as possible (even if a couple of puffs of a short acting beta-agonist are needed). These two PEV markers will be referred to in the subsequent sections.

MILD INTERMITTENT
Many asthma patients fall into the mild intermittent category. This group of patients may be symptom free for extended periods of time or may have short exacerbations on a fairly frequent basis. To be classified as mild intermittent a patient will have symptoms such as wheezing or shortness of breath no more than twice per week and nighttime symptoms no more than twice per month. The symptomatic exacerbations may last from a few hours to a maximum of a few days (although the severity may vary from one episode to the next). Between episodes there will be no symptoms and lung function tests will be normal. In this group lung function tests will show a PEF that is at least 80% of the predicted (best afternoon) value and have a variability of less than 20% (between morning and afternoon). This type of asthma patient usually will not require medication on a daily basis and can use a short acting rescue inhaler such as albuterol if needed for symptomatic control. A rule of thumb is that if the rescue inhaler is used more often than twice per week or if a canister lasts less than a month then there may be need for some type of controller medication. An occasional flare-up can be treated with a short course of steroids such as prednisone. A special class of asthmatics should be mentioned here, these are those with exercise induced asthma. A patient with exercise induced asthma typically will only be symptomatic during times of physical stress and usually can be controlled by pretreating with a short acting inhaler such as albuterol or even cromolyn. A diagnosis of exercise induced asthma although often easy to control should not be taken lightly for without pretreatment to prevent symptoms an attack could become a medical emergency.

MILD PERSISTENT
This class of asthma presents with patients who have symptoms more often than twice per week but less than once per day. Mild persistent asthmatics often have nighttime symptoms more often than twice per month but less than once per week. Lung function testing would show a PEF of greater than 80% of the predicted value which is similar to mild intermittent but with the difference of more variability in the 20 to 30% range. Most mild persistent asthmatics can be best treated with inhaled corticosteroids with a rescue inhaler used only on an as needed basis. Other treatment options exist but will not be covered here. This is the class of asthmatic that seems to often be mis medicated because although a rescue inhaler will often keep many patients essentially symptom free it will do nothing to decrease the inflammation that is a component for even the mild persistent asthmatic. This point should be reinforced: you do not treat persistent asthmatics with a short acting inhaler as mono therapy and the rule of thumb should be considered and a patient considered not under suitable control if they exceed one inhaler per month.

MODERATE PERSISTENT
Prior to treatment the moderate persistent asthmatic typically has daily symptoms with exacerbations at least twice per week on average. These flare-ups affect normal daily activity and often last for a number of days. Nighttime symptoms are seen more often than once per week. Lung function tests will show a PEF in the range of 60 to 80% of the predicted value with a variability of greater than 30%. Like the mild persistent asthmatic there are many moderate persistent asthmatics that are not being treated correctly. A short acting rescue inhaled used as monotherapy for an asthmatic at this level is simply bad medicine. It must be remembered that any one of the classifying symptoms is enough to place a patient in a given level so for example if a patient has nighttime symptoms more than once a week (one of the features of this class) then they should be considered to be moderate persistent even with out any of the other features. Remember if uncertain where to stage a given patient the physician should move in the direction of higher rather than lower classification. The moderate persistent asthmatic is usually best treated with a low to medium dose inhaled corticosteroid in combination with a long acting beta-agonist. Other treatment options exist but this is the best for most moderate persistent patients. Once again it needs to be reinforced that excessive use of short acting inhalers on a regular basis is a sign of poor control and the need for reevaluation of the treatment plan!

SEVERE PERSISTENT
This is the highest classification of asthma patient. The severe persistent asthmatic is always symptomatic with the ability for only limited physical activity. Both daytime and nighttime exacerbations are frequent and can last for extended periods. Lung function testing will show a PEF of 60% or less of predicted value with a variability of greater than 30%. The severe persistent asthmatic is usually best treated with a high dose inhaled corticosteroid combined with a long acting beta-agonist. To achieve long term control oral corticosteroids are often needed with the goal of achieving control with the lowest daily dose possible thereby reducing systemic side effects.

The reason it is not a picture of a nice house, or a good job or a wonderful relationship is because you refuse to draw your own picture. We convince ourselves that if we do not have it, there is no use in drawing it.

The fact of the matter is that you do have whatever IT may be. The canvass is all things, merely waiting your brush stokes to make it real. When you say that you have nothing to be thankful for, you are saying that the canvass can never have the picture you desire. You close the door without even trying to open it. You see the canvass as blank, but it can be whatever you desire it to be.

Giving thanks means that you have already received something. When you give thanks to the Universe for all that you could possibly desire, you are saying that you have received all that you could possibly desire. You give form and then substance to the energy that exists everywhere and by law, these things must manifest. If you say that the picture on the canvass is a new house, it cannot be a run down apartment-the panting can only be what is put on it.

It is only hard to give thanks for what you think you do not have because that is your conditioning. You can't paint? Not true-the world you have, you have painted. When you begin to accept the fact that you are the artist of your own life, you will begin to realize that all of those blank canvasses await your secret desires to be drawn by your creative mind.

Give thanks for all that you desire. They already exist in potential-it is up to you to make them real. TELL the Universe what you want and tell the Universe you are thankful that it has given you an unlimited supply of paint. Do not close the door of life by saying no thank you. Whether you are not able to fall asleep, wake up too often, or don't feel well-rested when you wake up in the morning, my guidelines will provide you with various useful techniques to improve these problems. Some of my recommendations include:

• Listen to White Noise or Relaxation CDs. Some people find the sound of white noise or nature sounds, such as the ocean or forest, to be soothing for sleep. An excellent relaxation meditation option to listen to before bed is the Insight audio CD.

• Sleep in complete darkness or as close to it as possible

• Turn off the TV before bed and read something spiritual or religious

• Avoid using loud alarm clocks

• Wear socks to bed

Most U.S. States and Canadian Provinces have their own laws on the requirement for brakes on a towed trailer. The word trailer also applies to a vehicle being towed behind a motor home. These laws are normally based on the amount of weight being towed. One problem with this is that it might be legal to tow a 2,000 pound trailer with no brakes in the state where you live, but as soon as you cross the state line of a bordering state it is illegal to tow the same trailer without brakes. Add to this your insurance company may not cover you in the event of an accident involving a trailer with no braking system. The brakes on a tow vehicle or motor home are designed by the vehicle manufacturer to stop the weight of that particular vehicle, not the additional weight being towed behind it. This additional weight adds a substantial increase to the distance required to stop safely.

Travel trailers and 5th wheels come equipped with electric brakes and RV dealers educate the RV consumer on the requirements for adding an electronic brake control to the tow vehicle. Some pop-ups are ordered with brakes and some without. Again, this is often times based on the braking laws for the state you live in. Keep in mind that once you cross a state line it could become illegal. My advice is if youre going to purchase a pop-up get one that has brakes.

The real culprit for disregarding braking laws is a motor home towing a vehicle behind it. Most people assume that because of the size of the motor home there is no need for a supplemental brake system on the towed vehicle and sometimes RV dealers fail to communicate the requirement for a supplemental braking system.

I for one was guilty of towing a vehicle without a braking system for quite some time. A close call, while driving through a major city during rush hour, suddenly educated me on how important it is. Another thing that surprises people is how much the vehicle theyre towing actually weighs. Take your towed vehicle to a set of scales and have it weighed. Make sure it has everything in it that will be in it when you are towing it. After you weigh it double check to make sure the receiver on the motor home is rated to tow that amount of weight and that you arent exceeding any of the motor home weight ratings like the Gross Combined Weight Rating (GCWR). The GCWR is the maximum permissible weight of the fully loaded motor home and the fully loaded towed vehicle combined. Most RV chassis manufacturers base their GCWR on the assumption that a supplemental brake system is being used.

Regardless of the brake system that best suits your individual needs the important thing is that you have one. There are many reasons to have a braking system on your towed vehicle.

But, most importantly it SAFELY reduces the stopping distance and helps to protect you, your loved ones and the safety of others.

If you are like most people online you have seen the offers for traffic. It seems that everyone and his brother has "unlocked the secret" to getting huge amounts of traffic to your website.

But is that true? Are those offers real? And if they are real why are so many people still desperate to get enough traffic to their sites?

The fact is - all traffic is not created equal. And if you think "hits" to your site will do you any good you might be shocked by what you read next.

You see, there are two sources of traffic. One is visits by real people who are interested in what you do. The other is what some call "raw" traffic, which is simply a visit to your site by a computer robot.

I'm sure you know that the first type of traffic can make you rich while the second will leave you disappointed, disillusioned and (usually) with much less money in your wallet.

The next thing you need to know is this - there are only two ways to get real traffic. You either buy it or you earn it.

Now I'm going to assume for this article that you are not made of money and can't spend $1000 a day buying traffic from Google. So let's look at how you can earn some real traffic - remember by real traffic I mean real people who want to know something more about what you offer.

There are four key factors when getting real people to visit your site. Happily, they have nothing to do with keywords or search engine optimization or cloaking or any "white hat" or "black hat" tricks.

They just work . every time!

KEY FACTOR 1 - Involve people, not robots. If you had ten friends who sent 10 friends to your site how much traffic would you get? If you answered 100 visitors you are only partially right. Let me explain.

When people visit a site they generally do one of three things. They buy, they bookmark or they leave. It has always been so.

That means that the 100 visitors you get today could well return. Assuming that 25% of them do return (which is conservative) then on day two you would get 125 visitors. And so on it goes. When those 125 tell their friends, magic happens.

Just imagine having 20 friends send traffic your way! Or having 10 or your friend's friends sending you traffic.

This viral marketing is the power of the Internet. Be sure to put it to use in your business.

The key to getting real people to send traffic to your site is that this traffic builds and lasts. Very important.

KEY FACTOR 2 - Offer something people want. What do you want? I'm willing to bet that you want more real people to visit your site, which is why you are taking time to read this article.

Guess what - that's what everyone wants. Everyone wants more traffic.

Consider this. What if a good friend of yours, someone you trust, said to you that they could give you all the traffic you wanted to your site and it would only cost you 10 minutes of your time. Would you check it out?

Sure you would.

When you begin to offer people what they really want they will visit your site in record numbers. Selling is hard but giving is easy.

What can you give that people want?

KEY FACTOR 3 - Immediate gratification. Offering something people want is one thing. Giving it to them now is something else.

In what ever you do, be sure there is an element if immediate gratification. Offer something that people can do right now, something they can benefit from in the next 10 minutes. When you do that you will see your traffic numbers rise dramatically.

What if there was a way for people who know and like you to recommend you immediately? Or to act on your advice quickly?

Review how you get traffic, or try to, today and ask yourself "is this working quickly enough?" If the answer is no, keep searching for a quicker solution.

KEY FACTOR 4 - Leverage - One effort creates many actions. This is huge. When it comes to generating traffic for your website you simply can not afford to spend all day every day placing ads or posting to blogs or any of that.

What you must do is find a way to do the work once and reap the benefit many times.

Think of it this way - would it be more effective to build a site and get 1000 visitors or to work with a site that gets 10,000 visitors? If you answered with the latter, you understand leverage.

You don't need to reinvent the wheel to succeed. By partnering with sites that have traffic now you can get all the traffic you want and begin to build your own powerful web presence.

Using the tips above I believe you will discover that getting more, and better, traffic just might be easier than you think!

How can you "break the ice" with a youth who is a total stranger and guide the discussion from an awkward silence into a meaningful conversation?

Jesus often got people talking with his first question and then was able to guide the discussion so that it developed into a meaningful conversation about a life-changing topic!

He met people where they were--whether they were drawing water, fishing, collecting taxes, watching him from a treetop, or worshipping God in the Temple in Jerusalem.

But Jesus was never content to leave them where they were. His objective was always a changed life!

Remember the Woman at the well... A simple request for a drink led to a meaningful discussion on "living water" that transformed a village.

Zacheus was simply hoping to get a glimpse of Jesus but instead got to talk to him at his own dinner table in a conversation that not only changed his life, but reached out to the lives of many sinners!

Christ's first words to Nicodemus, a religious scholar, were a conundrum, a puzzle that left him searching. And that search seemingly reached its conclusion when Nicodemus took a bold step of faith, along with Joseph of Arimathea, and asked for the body of Christ after the Crucifixion.

The gospels are filled with the stories
of people who encountered Christ and then
their personal stories would be forever changed!

In encounter after encounter... Jesus not only broke the ice, but he left a lasting impact on the lives of the people he spoke with!

Sometimes Jesus used questions to break the ice. But on other occasions he use everyday objects, shared experiences, stories, and even crises to break the ice and open up the way to life changing spiritual discussions.

He asked people to share their opinions,
challenged their assumptions,
and asked them to find solutions.

He constantly invited others to join him in experiences
and used those common experiences
to bring out life changing spiritual truths.

Fresh experiences often served as living parables.

It's easy to relate to the feelings, thoughts, and experiences when you have just experienced them. Then it is only a short little jump to deeper truths and understanding.

The "here and now" becomes a springboard
to the distant future,
to choices and consequences.
A small controlled simulation
becomes a safe crucible
to test the consequences of real life decisions.

In the same way today, games, challenges, and contrived situations can generate discussions and expose the same deep set beliefs, attitudes, and values as real life situations and events.

How you play the game
is a reflection of who you are
and how you deal with life!

But in all things Jesus always had a purpose.
He broke the ice,
and later his own body would be broken,
so that broken lives could be mended.
