You can have a bachelors degree, masters degree, PhD; you can be a CAC, LPC, NCC, MD, or any of the other letters that define us clinicians, however if you cant create a genuine relationship with a patient/client you will not be an effective clinician. I have also observed patients reactions to these counselors and I must say that they were not positive or healthy responses. I never understood the reason for this behavior, however I suppose it is the clinicians own stuff that they have not effectively worked on. I often confront this behavior and attempt to change it, although many times I fail. As clinicians, I believe we need to look inside and gain insight into our behaviors with our clients and reexamine our reason for working in this field. We need to ask ourselves why we have the need to feel better than, why we need to be rigid and control our clients, why we need to demand things of our clients, and most importantly do we genuinely care for our clients well being. We must reexamine our thoughts, feelings, and behaviors often in order to be the best helpers we can be. After all there are many other jobs out there if you truly do not have your heart in this profession. Its perfectly fine to move on and in fact more respectful than working in this field without the passion for the patient.

There is so much research out there that tells us that the therapeutic relationship is vital in treatment outcomes and I continue to see counselors treating clients in the absence of a strong therapeutic alliance. You can be an expert in theory, technique, research, and whatever else you choose, however if you cannot develop a strong therapeutic bond the clinical effectiveness of your treatment is at best questionable. The foundation of therapy or counseling is a strong therapeutic working alliance. There is plenty of research out there that confirms this.

My recommendations to address this issue that I have discussed are as follows:

1. Continue to monitor your thoughts, feelings, and behaviors regarding the therapeutic relationship.

2. Continue to work on your own personal growth through 12 step recovery, personal therapy, retreats Etc.

3. Review and continue to practice the basics of counseling therapy, such as rapport, trust, empathy, positive regard, etc.

4. Examine your true reason for being in the addictions field. Do you care? Are you non judgmental in your approach? Are you in it to feel powerful, controlling, superior?

5. Be a real human being with your patients!

You may or may not agree with the issues I have just discussed, however I am passionate about my career and get extremely frustrated with clinicians and staff that are inconsiderate, act superior, and demoralize patients. I have entered this field because I am a recovering alcoholic and addict that want to help others to enjoy a better way of life. My one and only priority is the patient. I hope that all clinicians reading this will take an in depth look at themselves as well as share this with other clinicians. After all, our purpose is to help other human beings just like ourselves to enter the exciting journey of recovery and leave behind the destruction and despair of addiction. Some people say I have the best job in the world. Sometimes I even think so myself. My job, in addition to teaching sailing, directing the sailing school program at Spinnaker Sailing in Redwood City and brokering charter vacations is to organize and lead group flotilla charters on behalf of Spinnaker Sailing. This has been going on three or four times a year since the late 1980's.

Our most recent trip was to The Grenadine Islands in the Windward Antilles from May 2 to May 11, where we chartered 4 boats from Sunsail in St. Vincent. We were 19 people in all making comfortable boat loading of 4 to 6 people on boats ranging from 45 to 50 feet in length. The daytime temperatures were in the mid 80's and the wind was steady in the 20 knot range just slightly north of east. My boat was an Oceanis 461 which also carried my wife, Melinda, Harry Gull who has done numerous trips with us and Paul Disenso, a student going for his ASA Bareboat Charter certification during the trip.

There are several bases for a Grenadines charter both inside and outside the immediate area. Bases outside the Grenadines in the north are in St. Lucia and even further in Martinique. It's a glorious reach south into the Grenadines from the north. The distance is about 70 miles from The Moorings base at Marigot Bay in St. Lucia (north of St. Vincent) to Bequia in the northern Grenadines. Getting back isn't so easy and many charterers choose to continue one way to Grenada to finish their charters. If you choose to start in Grenada where both Moorings and Sunsail have bases, it's 40 miles (mostly upwind) to Carriacou in the southern Grenadines. Between Carriacou to Bequia it's generally reaching with gaps between the islands of less than 15 miles. Returning to Grenada is another glorious sleigh ride.

For those who don't like or don't have time for long passages, there are currently two places to charter right in The Grenadines. The Sunsail base is at the Blue Lagoon Marina at the southern end of St. Vincent. From there it is only 8 miles into Admiralty Bay in Bequia. Also, The Moorings has recently opened up a base in Charlestown Bay at Canouan Island right in the middle of the Grenadines. From California, both places can be reached by 3 flights which means getting to either base is almost as easy as getting to B.V.I. We wanted a charter location where we could enjoy as much time as possible in The Grenadines. St. Vincent fit the bill quite well.

After a mid-afternoon arrival in St. Vincent and a night on the boats, Sunsail provided us with boat and chart briefings. Before setting sail on a bareboat vacation there's more to do. There's checking and stowing the provisions and checking out the boat and all the equipment. Sunsail has a provision and beverage ordering system via the internet that is reminiscent of ordering from Webvan. The cold and frozen stuff was bagged up and waiting for us to pick up in the market at the marina and the rest was aboard the boats when we arrived. It was mid-day before we set sail for the short passage to Bequia on an easy reach.

Flavours catch fish period. But how many catch more fishermen than fish? The idea of a flavour dispersing through the water and attracting fish following its trail and increasing concentration to its source and finding your hook baits, is taken for granted now. How many anglers have not heard of the world famous Tutti Fruitti flavour?

When fishermen think of fishing bait flavours they often think of the ester, ethyl alcohol or solvent based types which were originally familiar and popularly used in baits the original dough, paste and boilie mixtures of the past. Such flavours as strawberry and vanilla are still used by anglers making their own baits, even today.

However, today many flavours are vastly more than combinations of esters for example, that have proven by trial and error to catch more fish than many others tested. Even the powdered flavoured ingredients like the milk shake powders are flavours containing many different components.

Some flavours are actually far more soluble, actively mobile and more volatile in water than you might imagine and extremely powerful carp feeding triggers in their own right. Some perform far better in summer than winter. Some stand out even really producing more fish with high pressure and low water temperatures.

Some of the very few most well known original top flavors are even enzyme active and can contain extremely low levels of very special chemicals which would actually be poisonous if used in higher levels. But then this is the case with most of the powerful mind-altering drugs known today.

There are many secrets of flavours well beyond the scope of any but the biochemist or marine biologist. The key point about flavours is that the very best can actually change the, condition of various nerve pathways sending signals to the brain, fish brain chemistry, mood and behaviour to one of; lets move and look for food and eat right now!

Here is one basic example before we go into an introduction into fishing flavours. The extremely sweet protein called Talin is very significant in connection with nerve pathways and special signalling gating mechanisms which in the end affect brain activity and fish behaviour for example. Talin is an integral part of many modern specialist carp bait flavours with good reason. But more of this advanced stuff later...

Certainly in the UK and Europe, flavourings seem to be inextricably linked especially to carp baits and I think this is largely due to the following reason:

In the early 1980s era readymade boilie baits for carp fishing baits were really only just hitting the market with Richworths right at the start of the big commercialization of carp fishing. Just as carp fishing popularity was about to really explode, along came one particular method.

Basically it was fishing a bolt rig set-up with a highly flavoured low nutritional boilie bait which often was a popped-up bait which was totally revolutionary at the time for the vast majority of the anglers.

Only a few of us who had thought of using dog biscuits off the lake bottom, perhaps even directly off the lead, were using anything similarly effective.

A back-stop was used on the line to stop the fishing weight from running up the line as a fish picked up the bait and moved off with it in its mouth. This was very effective at hooking the fish as the line running freely through the lead weight on the line stopped suddenly at the back stop and jerked the hook firmly into the fishs mouth.

The availability of the new readymade highly flavoured boilies and the very effective combination of a heavy lead, hair rig, back stop and better rig materials and hooks like the chemically sharpened Kamasan hooks made this a very easy period to take up carp fishing with high expectation of easy quick fish rewards.

The readymade baits were of a kind of neutral nature as the base mix did not carry a flavour and appeared quite bland. In fact these baits were even available in unflavoured form so that you could add your own choice of flavour to them by soaking neat flavour directly into them and leveraging the freezing in and absorbing effect of defrosting baits.

Or you could simply put some flavour into a bag of neutral baits, shake it about to disperse the flavours and go fishing. Its just the same today when you want to flavour a bag of pellets.

The effect went further in that you could now buy many of the flavours used in the readymade boilies themselves. With these you could add even more flavour to really over flavour your baits or make interesting alternative or unique bait flavour combinations. You could buy readymade floating or pop-up baits in various flavours too.

Pop-up baits were used to fish the hook and bait off the lake bed to make them even more obvious and enticing to the fish. Also in all probability, these were very likely to be taken first if you had baited your swim with lots of free baits to attract the fish into the swim. This method really multiplied catches.

These baits were a revolution in themselves and literally ripped carp waters apart at the time and where they had not been used before.

At this time most fishermen were coming from an age where you had to make your own baits whether boilies or pastes, because there was simply no alternative, although there were already more and more boilie base mixes coming onto the market for making homemade baits.
